Contract
Please
read carefully. you
will be asked to sign this contract at the Studio 10 office. Be sure
to retain a copy for your records, as these policies will be strictly
followed. Exceptions or changes cannot be made for any reason.
Registration
Fee
The
undersigned, as a guardian of the registered, above listed student,
makes application on his/her behalf for acceptance into classes at Studio
10 Dance Center. I understand a $45.00 non-refundable
registration fee must accompany this form.

Dropoping a Class/Classes & Refund Policy

Studio
10 Dance Center is a Year-Round Dance Center (August '05 - August
'06). To terminate enrollment, drop a class, or make changes, it
must take place at between January 1st - 7th, 2008with
ALL TUITION PAID IN FULL through the end of the semester. The only
other time you may drop from classes at Studio 10 Dance Center or
drop a class from your schedule, is at the end of the spring semester
(June 30, 2008) with ALL TUITION PAID IN FULL through June 30, 2007,
even if the student does not attend classes through June 30, 2008.
- No
refunds are given for tuition, registration, end of the year concert
fee or costume fees.
Tuition
& Fees
- As
Guardian of the registered student, you are responsible for all tuition
for each moth from August 1, 2007 or from the time of enrollment through
June 30, 2008, unless the "Drop Form" is given to the office
at the above specified times, even if the above listed student does
not attend theses classes. The $45.00 registration fee is required to
be paid again if a student withdraws at anytime and returns thereafter.
- The
preferred method of payment is automatic deduction from your checking
account (forms available in the Studio 10 Office), credit card on file,
or payment in full for the entire year (discount will apply.) Receipts
are not given for checks. Studio 10 Dance Center does not have change,
therefore when paying cash please have the exact amount and be sure
to receive a receipt. Online payments are also acceptable.
- If
paid monthly, Tuition is due the first class of the month. If tuition
is not received by the 7th of each month a $10.00 late fee will be added
to your account and after the 20th of each month an additional $15.00
late fee will be applied. Any balance not paid in full
by the end of the month receives a 10% charge applied to the remaining
balance each month until the balance is cleared. If the tuition is not
received by the end of the month, the student will only be allowed to
observe class and will not be allowed to participate. We will allow
the student to call his/her parent to rectify the situation. After two
(2) consecutive months of non-payment, the student cannot enter the
class(es) and will be required to repay the $45.00
registration fee upon return.
Insurance
- Studio
10 Dance Center does not carry medical insurance for its students. It
is required that all students be covered by their own family insurance
polices. If injury occurs, it is understood that the student's own policy
is your only source of reimbursement.
- As
a condition of acceptance in dancing classes, I hereby covenant and
agree to hold Pat Holbrook and Studio 10 Dance Center harmless and forever
indenmify it, its owner, and its agent from any and all liability arising
from injuries which my child should sustain while on the premises occupied
by the school.
The
policies of Studio 10 Dance Center will not change or exceptions cannot
be made by Pat Holbrook or the office staff, as these policies are set
in place to ensure the smooth and professional operation of Studio 10
Dance Center.
I
understand that upon enrollment in the Studio 10 Dance Center, I have
read, understood, and agree to abide by all of the school policies while
attending classes at Studio 10 Dance Center THERE WILL BE NO REFUNDS FOR
ANY REASON. Upon re-enrollment of the above listed student in subsequent
classes, I understand and agree that the condition above will be applicable
to such reenrollment and binding on me without the necessity of completing
another enrollment contract.
We
reserve the right to refuse service and terminate enrollment.
